I was looking for viable jotform replacement and have been evaluating all the open source tools available and along with some paid ones. All of them where either missing features I needed or where quite expensive.
FormDesigner has been able to fulfil all my needs, including writing into a MySQL DB directly.
Questions I had have been answered by the dev in a timely manner.
I'm really happy with the product and can fully recommend it!
