I am looking to create a form where the response to some questions/labels will require the responder to enter a list of two or more options. For example, I want to have a field with the label _"Shareholders Details" - which will include name, address, tel, and email. There may be two or more shareholders. How can I implement this by enabling the responder to click a tab to add each subsequent shareholder?
The headline "Multi-Item List Option" and the text confuse me a little. If I understand correctly, you are asking for a "repeater" field. So essentially a field, or in your case a group of fields, that is created once while creating a form, which then can be added multiple times by the respondent by clicking an "Add more" (or something like that) button... right?

I'm a heavy user of tally.so, their UX is really sleeck, and works fantastically : you can control the form behaviour and apparence from the form construction space itself, with simple '/'. On the other hand, at the end of the day with deftform it seems that the majority of time is spent in menus ... which isn't "sleek".
Are you aware of them, and would you consider an tally-like interface?

We want to use deftform for new user signups through PushLapGrowth affiliate program (another appsumo deal) . For this we need to inject one script in <HEAD> part of the HTML and another script in the <BODY> part of the HTML. Currently with deftform it is possible only to inject one script into <BODY> part of the HTML. We would really appreciate if you could help.

I saw you mentioned about creating custom chart. Instead of creating custom charts (from the form), why not you also include features just to show the data in "table" form? It is simpler and easier to implement.
